Abstracts for parallel talks and posters should be submitted through the web site (although the form to do so is not yet ready). REGISTRATION The registration fee will be $325. There is a discount rate for full time Ph. D. students. Registration will take place with forms available from our website http://lqcd.fnal.gov/lattice2004/. There is a separate registration form for students AND another one for employees of the US Department of Energy and DOE contractor-operated laboratories (e.g., Fermilab, Argonne, Brookhaven, Jefferson Lab, LANL). Please use the appropriate form. If you have no web access, please e-mail us at lat2004@fnal.gov If you are not a U.S. citizen and do not hold a current photo ID badge from Fermilab or another DOE installation, you must, for security reasons, fill out another form after completing the registration form. We apologize for the inconvenience and the personal nature of some of the questions. An advantage of this form is that it will enable us to send out invitations to participants who need one to secure a visa or travel support. Participants are expected to obtain their own funding. We are applying to the US National Science Foundation for support for students who are unable to obtain sufficient support elsewhere. We also have a small fund to support speakers from developing countries. Please circulate this e-mail to your colleagues and collaborators, particularly to those who did not attend last year or have changed e-mail address since then. WELCOME A welcome reception will take place in the evening, Monday, June 21, in the symposium hotel Holiday Inn Select Naperville Hotel 1801 N. Naper Blvd. (intersection of Diehl and Naperville Roads, near Interstate 88) Naperville, Illinois Please attend and visit the registration desk during the reception. It is important to do so, because it will provide you with a symposium pass allowing you access to Fermilab during the symposium. The scientific program will begin on the morning of Tuesday, June 22, and will conclude around noon on Saturday, June 26. For those of you leaving late Saturday or on Sunday, tours of Fermilab will be arranged for Saturday afternoon. VENUE The symposium will take place in Wilson Hall, Fermilab's high rise. There is a public e-mail center on the ground floor of the building, and wireless internet throughout. HOTELS Blocks of rooms have been reserved in the Holiday Inn Select Naperville Hotel, the Best Western Naperville Inn, and the Exel Inn (Naperville). They are close to each other and within walking distance of some restaurants. Participants must make reservations directly with the hotels. For contact information and codes for the symposium discount, consult the Travel/Hotel section of the web site. Hotel reservations must be made by June 7, 2004. Bus service from the symposium hotels to Fermilab (and back) will be provided. TRAVEL The closest airport to Fermilab is Chicago's O'Hare Airport, with non-stop flights from many cities around the world. American travelers may also find Midway Airport useful; it is only slightly farther away and sometimes is a better bargain. To get from the airports to Fermilab, we recommend limousine service or rental car.
